Leto began dating actress Cameron Diaz in 1999, and they became engaged in 2000. Their relationship ended in 2003.

He became a teetotaler in 2007.

Leto moved to Los Angeles in 1992 and began appearing in television programs, including Camp Wilder, Almost Home, and Rebel Highway. His breakthrough came with My So-Called Life (1994-1995), in which he played Jordan Catalano, the love interest of Claire Danes's character, Angela Chase.

His first theatrically released film was How to Make an American Quilt (1995). He later appeared in The Last of the High Kings, Switchback, Basil, and Urban Legend. In Prefontaine (1997), he portrayed Olympic distance runner Steve Prefontaine. To prepare, he trained for six weeks and met members of Prefontaine's family and circle of friends.

Leto followed these roles with supporting performances in Terrence Malick's The Thin Red Line, Girl, Interrupted, and David Fincher's Fight Club. He also played Paul Allen in American Psycho (2000).

Leto received critical acclaim for playing heroin addict Harry Goldfarb in Darren Aronofsky's Requiem for a Dream (2000). He lost approximately 28 pounds for the role and spent time living on the streets of New York while preparing for the film.

He continued taking physically demanding roles, including Mark David Chapman in Chapter 27 (2007). Leto gained about 60 to 67 pounds to portray John Lennon's killer, and the rapid weight gain caused gout and forced him to use a wheelchair during part of the production. He later played several versions of the title character in Mr. Nobody (2009), including an elderly version requiring extensive makeup.

After a five-year break from filming, Leto returned as Rayon in Dallas Buyers Club (2013), directed by Jean-Marc Vallée and co-starring Matthew McConaughey. He lost 30 pounds, shaved his eyebrows, and waxed his entire body for the role. His performance earned him the Academy Award and Golden Globe for Best Supporting Actor, as well as a Screen Actors Guild Award.

Leto later played the Joker in Suicide Squad (2016) and reprised the role in Zack Snyder's Justice League (2021). His subsequent film credits include Niander Wallace in Blade Runner 2049 (2017), Albert Sparma in The Little Things (2021), Paolo Gucci in House of Gucci (2021), Dr. Michael Morbius in Morbius (2022), Ares in Tron: Ares (2025), and Skeletor in Masters of the Universe (2026).

He also portrayed WeWork co-founder Adam Neumann in the limited television series WeCrashed (2022).

Leto formed Thirty Seconds to Mars in Los Angeles in 1998 with his older brother, drummer Shannon Leto. Jared Leto serves as the band's lead vocalist, multi-instrumentalist, and principal songwriter. The group's debut album, 30 Seconds to Mars, was released in 2002, followed by A Beautiful Lie (2005), This Is War (2009), Love, Lust, Faith and Dreams (2013), America (2018), and It's the End of the World but It's a Beautiful Day (2023).

A Beautiful Lie brought the band wider public attention and was certified platinum in the United States. The band entered the Guinness World Records in 2011 after performing 300 shows during a single album cycle. Its album America debuted at number two on the Billboard 200, the group's highest position on that chart.

Leto directed many of the band's music videos under the pseudonym Bartholomew Cubbins, including videos for "The Kill," "Kings and Queens," "Hurricane," "Up in the Air," and "City of Angels." The video for "From Yesterday" was filmed entirely in the Forbidden City in China.

Leto made his directorial debut with Artifact (2012), a documentary about the music business and Thirty Seconds to Mars's legal dispute with record label EMI. The film premiered at the Toronto International Film Festival and won the People's Choice Award for Best Documentary.

He also directed the documentary series Into the Wild, which followed Thirty Seconds to Mars during its concert tour, and the documentary A Day in the Life of America. In 2011, he launched VyRT, an online platform combining live video streaming, social networking, and merchandise. VyRT received the Best Online Concert Experience award at the 2012 O Music Awards.
